Same work, different provincial vocabulary
Canada has never agreed on a single name for this role. Alberta says health care aide. Nova Scotia and Saskatchewan say continuing care assistant. Ontario says personal support worker. British Columbia says health care assistant. The daily work behind each title is broadly the same: personal care, mobility and transfers, mealtime assistance, observation and reporting, and supporting people whose independence is changing.
Because the names differ, so do the curricula and the hours. That is the real reason a certificate does not transfer automatically. It is not that one province distrusts another; it is that nobody can assume two programs taught the same content when they were never written to the same standard.
What Alberta actually asks
Practice in Alberta is regulated by the College of Licensed Practical Nurses and Health Care Aides of Alberta. If your education was completed under a different provincial title, the route is an assessment of whether your training is substantially equivalent to the Alberta curriculum. A favourable assessment does not exempt you from the examination. The Alberta HCA Registration Examination is how the province satisfies itself that a registrant meets its own standard, whatever the certificate says.
The practical consequence is that a continuing care assistant moving to Alberta should plan for two things at once: the paperwork of assessment, and preparation for a one hundred question examination sat under a two hour limit. Treating them as sequential is how people lose a season.

Alberta is precise about what a health care aide does. The competency profile says a health care aide provides medication assistance as assigned by a regulated health care professional. An examination option that has the aide administering, adjusting or deciding a medication is a wrong option, every time, and recognising that at a glance is worth marks. If your previous role used looser language, this is the habit to unlearn first.
Reporting rather than assessing
A health care aide observes, reports and documents. Assessment and clinical judgement belong to a regulated professional. Experienced assistants often answer as though they may decide, because in practice they have been trusted to. The examination measures the boundary, not the trust.
The province's own terms
Alberta uses responsive behaviours for the changes in behaviour that accompany dementia and delirium. Candidates who trained elsewhere often arrive with a different vocabulary and read past the right option because it is worded unfamiliarly.
How to prepare when you already do the job
Experience helps with the content and hurts with the format. Applying carries between 53 and 73 per cent of the marks, so most questions are situational, and a person who has done the work will often recognise several options as things they have genuinely seen done. The question is not what happens on a real unit; it is what an Alberta health care aide should do within scope. Practising against that distinction is the difference between a comfortable pass and an unpleasant surprise.
